So, you’ve done your homework and decided to go with one of the new dental or vision plans available. Don’t go looking for an enrollment form: there isn’t one.

Instead, the ONLY way to sign up for the programs is online or by telephone. (877) 888-3337, TTY (877) 889-5680

BENEFEDS.com tells the provider you’ve signed up, calculates your deduction amounts, and takes care of asking your agency to make the deductions.
